Stages of Transition

Transition can be separated into three stages*.

For the purposes of this training the stages will be called:

• Beginning

• Intermediate

• Advanced

Youth and young adults will be developmentally ready for stages at different ages. Planning and activities for transition should be individualized to the youth and young adult.
